South Creek Post Acute’s facility is located in Centralia in the state of Washington. We are a 128-bed facility dedicated to providing top-quality post acute care to our patients.

At South Creek Post Acute, our mission is to be a skilled nursing and rehabilitation center that provides comfort, support and care in a home-like setting. South Creek Post Acute specializes in speech pathology to help seniors regain the ability to speak clearly, process information, and safely swallow food and liquids to prevent complications like pneumonia.

Our Team

Our skilled nurses, therapists, caregivers, and support staff work together to provide the highest quality of care to every resident.

Leadership Team

Our Licensed Nursing Home Administrator (LNHA) directs all South Creek Post Acute operations, staffing, services, and programs. This leadership team also includes our Medical Director, who coordinates medical services according to policies and procedures, as well as our Director of Nursing, Rehabilitation Director, Social Services Director, and Environmental Services Director.

Andrea Harris – Administrator

Professional Nursing and Clinical Staff

Led by the Director of Nursing, our nursing staff delivers care and clinical services according to quality standards. Registered Nurses (RNs) assess, plan, provide, and document nursing care. They oversee Certified Nursing Assistants (CNAs), who assist patients with activities of daily living including administering medications, eating, bathing, dressing, and transferring.

Therapy & Rehabilitation Staff

Our licensed Physical Therapists (PTs), Speech Therapists (STs), and Occupational Therapists (OTs) create and implement therapy plans according to the individual needs. Under the guidance of therapists, Restorative Nurse Assistants (RNAs) assist with regular activities to improve mobility and support recovery. This team is led by our Rehabilitation Director, who guides therapy and rehabilitation strategies, operations, and staff.

Denise West – Director of Rehabilitation

Social Services & Activities

The Social Services Director leads and plans programs that address the mental, social, and emotional needs of patients and families, and oversees Social Workers. Social Workers identify and coordinate individual services to maximize independence, dignity, and quality of life.

Nutrition & Dietary Services

Our Registered Dietitians (RDs) manage menu planning and meal service to ensure residents enjoy fresh, flavorful meals tailored to their dietary requirements. Every dining experience is thoughtfully prepared to support health, comfort, and overall well-being for those we serve in Lewis County

Environmental Services

The Environmental Services team oversees facility maintenance, laundry, general housekeeping, and facility cleanliness according to health and safety standards and codes.

Meet Our Leadership

Andrea Harris, Administrator

Andrea Harris is the Administrator of South Creek Post Acute, where she oversees the daily operations and leads a team dedicated to providing high-quality skilled nursing, rehabilitation, and long-term care services. Andrea began her love of helping seniors as a teenager where she volunteered at a local nursing facility. She then became a caregiver in a memory care facility. From there, she decided her biggest impact could be in healthcare administration. She obtained her bachelor’s degree in healthcare administration with an emphasis in Long-term Care from Weber State University and is passionate about improving the quality of life for residents and staff alike. Now with 18 years of experience in administration and 5 deficiency-free surveys, Andrea is committed to enhancing resident outcomes, supporting staff development, and fostering a resident-centered environment.  When she is not working, she enjoys spending time with her husband and two bonus children. They are avid sports fans and love their Seattle Mariners and Seahawks! 

Jett Cohen, Director of Nursing

Jett Cohen RN, BSN, is the Director of Nursing Services at South Creek Post Acute Care whose journey embodies the American dream. Originally from the Philippines, she came to United States with determination, resilience, and a vision of making a difference through nursing. Rising from CNA to LPN to RN, Jett built her career through hard work, leadership, and an unwavering commitment to excellence in resident care, staff quality of life and team development. Supported by her husband, a proud 20-year Navy veteran, she leads with compassion, integrity, and purpose, inspiring her team to deliver the highest standard of care. Happy to be here and proud to serve, she believes nursing is more than a profession, it is a calling and an opportunity to make a lasting difference in the lives of others.

Denise West, Director of Rehabilitation

Denise West, the Director of Rehabilitation at South Creek Post Acute, brings more than 15 years of experience in skilled nursing and post-acute care. A lifelong member of the local community, she has dedicated her career to helping patients achieve their highest level of function and independence through compassionate, patient-centered rehabilitation services. For the past 12 years, Denise has led multidisciplinary therapy teams providing physical, occupational, and speech therapy for short-term rehabilitation, long-term care, and outpatient populations. She is passionate about enhancing quality of life through holistic care, including initiatives such as developing a sensory room and leading a monthly community amputee support group. Outside of work, Denise enjoys spending time with family, staying active through outdoor recreation and endurance events, and traveling to explore new places, especially when her journeys provide opportunities to spend time with her daughter and stepson.
