I came to this facility from Harborview after receiving 4 weeks of therapy there. After 3 weeks here at South Creek I must say Denise and entire staff Jeremy,Alec,Jenny Jan,Katelynn,Kelly, and Laura are exceptional therapists. Equal to or above the therapists I had at Harborview. You have an exceptional staff. ALL THUMBS UP Sincerely Ken Wilson…

I’ve worked with 3 therapists….Lexus, Jenny and Jeremy. They are the people that have helped me the most during my stay here. They push you, but not beyond your limits. I love working with them! They will have me walking soon once my weight restrictions are lifted. I’m looking forward to that!! They get “kudos” from me!

The staff is grat

South Creek post Acute is the best healing center that you can heal in and it’s #49 out of 1500 for being the best in the United States. For the people like Katelynn, Alec, Kelly and all the other staff have been nothing but the best! They saved my life.

Pt and the Ot here was really helpful Kelly on Ot really was helpful on all most everything getting me ready for my journey home, Lexus was really helpful in getting my strength back to where I could walk again.all these people here were hands on and really eager to help one get back to their self the rehab was amazing I can’t say enough thankyou to all of them

Ive been a resident at other facilities and this facility is such an improvement with the quality of my care/treatment.very happy and the staff is excellent!

Mostly kind and knowledgeable staff! Very friendly as well. Good control of difficult residents. Could use some higher night stands. OT and PT successfully get through to us even if we are fighting them, they are great! Room is situated so we get plenty of daylight, able to have bird feeders outside our window!

The therapy department really cares and tries to get their patients back home and living their best and safest life possible. True professionals and not just people counting a number or doing a job but pursuing their passion for bettering lives.

This facility is great. It has plenty of activities, the staff is friendly and gentle with residents. They have popcorn days on Fridays (my favorite part)!

Enjoyed my stay here

My brother was at South Creek for 2 months. Initially the care was marginal but care change to neglect facilitated by lack of staff. When asked to correct simple tasks for activities of daily living such as assist out of bed, help with meals, water and consistent therapy the comment was “we are short staffed”. The neglect lead to a life threatening hospitalization which could have been avoided with care and supervision . I would avoid this SNF.

This place deserves no stars honestly if I could say zero I would say zero so within the last 24 hours I finally found out why everybody else in the community is not a fan of this place we had to pull my father out of this place just this morning 2 to the fact of him falling down twice while in the care of this place.seems crazy right, sure made my heart sink, To see my dad in pain and threatening to call the police come on now do you know better than to do that if you can’t even pay attention to your own personnel. Worst facility if you have other choices please go with them and not this Care facility

This is a wonderful facility. I actually was very worried when I had to leave my husband after he had a stay in the hospital. All of the staff took great care with my husband. The therapists went out of their way to find good timing for my husband when he was focused enough to participate. My husband got to recognize all of the staff working with him. They always got smiles from my husband and always gave smiles to my husband. Overall a very kind and caring staff. I appreciate all their hard work.

It’s a nice facility with very nice helpful staff. All my needs were taken care of promptly..Very clean and user friendly.. thx everyone for getting me up and on my feet again. My stay here was definitely a step in the right direction . Thx again.

Nursing leadership does a great job supervising staff. Everyone is very polite and they are resident-focused. Bravo!

Family member was put there from hospital without family knowledge. We were told they were given family info but received no call he was there. When admitted he was walking and after 41 days of what was supposed to be rehab turned out to be he was wheelchair and bedbound. They did not get him up and get him moving like they were supposed too. Even though he had a power of attorney, they refused to call the power of attorney and give updates after they said they would. They did not notify the family that he was going to be charged for his stay until he owed several thousand dollars. Maybe if they would have been in contact with his POA and told them after so many days in he will charged we could have prepared for him to come home. Calling that place is a joke as they never answer the phone. When he made the choice to leave because he couldn’t afford it, they called the sheriffs department on us stating that he was in danger of his family for him making his own choice to leave. Sheriff came to the house and made several attempts to break in even though it was obvious no one was at home. The facility called 911 before we were even off their property so it did not allow us to get home before the sheriff was there. We have tried to retrieve the clothes that were in the laundry when he left twice and no one will call us back. After 30 days they say they donate or discard the belongings. We have requested all of his chart notes and the pictures of his wounds so we could turn them into his doctor and he wants a copy for himself and we keep being told they will and they don’t. The first excuse was they didn’t have the emails to send info too which they had and then it was they needed the POA paperwork which they had had all along. I would not recommend this place for my loved ones. How do you say someone’s in their right mind and then turn around when it doesn’t profit you and say he cannot make his own decision to leave? This is not my first time having a loved one at a skilled nursing facility. Over the last two years this is the fourth one I have had a family member in and I will tell you it is one of the worst. Please, if you care about and love your family, do not put them there.

The staff was so helpful and knowledgeable, I was so blessed to have such a caring and supportive team available. If you’re looking for a safe and caring environment this is the place! Thank you for your kindness.
