On a recent weekend at South Creek Post Accute we had an emergency where we nearly lost a fellow resident. Immediately outside my open door resident Rickie, a big man, had fallen and was coming in an out of consciousness. A number of nurses and nurse aides, (CNAs) gathered around him, but CNA Shayna Lason took the lead in keeping Rickie conscious. It became clear to all of us that we were losing him. CNA Lason continuosly urged him to stay awake and awoke him a number of times when he slipped into unconsciousness. The tone, timber, and genuine determination in her voice kept him going until eventually the paramedics arrived. Even then CNA Lason kept him from pulling the needles out of his arm. Many that gathered around voiced their own encouragement, but it was clearly Shayna’s strong determination and compassion that Rickie responded to and was able to follow. As a Marine veteran it is this kind of care I would hope to find on the battlefield of life. Rickie is a fortunate man to be with us today. Thank you Shayna for being what and who you are. The physical therapy is top notch however the food leaves a lot to be desired

I have loved my experience here! The food is amazing, my favorite is the BBQ beef. The cooks are amazing. Scott always makes a great breakfast. My blood sugars have stayed stable my entire stay due to the kitchens attention to my diet. Raylee has even brought me hand written notes on my diet and how I can follow it once I go home. I love group exercise! Casie and Kaitlynn always make sure im included and encourage me. Therapy does a great job. Dan in social services is also very attentive and let’s me talk his ear off. I recommend this place to everyone! It’s clean, fun , and healing!!

Nurse Connie was great, Jake, Keaton and Alex are always upbeat, helpful, attentive and made me feel welcome. During the night Bente was really helpful, especially on my really tough nights. Kaitlyn in PT is encouraging and positively upbeat during every session. She would push but not past my ability. Overall everyone is friendly and professional.

Heard they had a first rate physical therapy team and they did not disappoint. But I found much more than.i found south creek full of caring hard working people..from day one I felt safe secure and cared for . I had so so many positive interactions with the staff. I am forever grateful for them. Starting with the backbone of the team,the cna’s. Alia is a hardworking lady that worked diligently to care for me and others in her charge. I. Bailey and her kitchen did a great job feeding me.loved the pizza an pot roast biscuits. Hard to cater to. So many different diets an needs but Railey an her staff did a good job.in house transportation got people to appointments thanks to Brooke and Rachel. occupational therapy gave me tools for my life at home.thanks Melissa Laura Kelly an a special thanks to jan..the administration was helpful whenever I neededit.thanks to Daniel for getting my in home nursing care set up for..thanks to shelbie with help with the financial stuff.thanks to Gracie to get me to write this review. Thanks tothe physical therapists Alec Jenny Jerry Ken Edward I hopeu learned a lot from Alec .Casey and Kaitlyn were a pleasure to be around. Denise you have a excellent team. .administrator Andrea you lead a great team like Jett Tammy Amanda Jill Casey conner alia becca.thats what makes South creek a excellent place to rehab heal both physically and mentally. The time went fast, the entire staff went above and beyond for me, made my stay as pleasant as possible, helped me in every way with my recovery, The nurses. CNAs, PT, OCC therapy speech therapist, fantastic, kind and polite, i want to name everyone, but i cannot.. just some are Danielle, Connie ,Gregory, Alivia, Brittany, Jake Alex,Keaton ,Michelle Maria Nancy,Melissa,Jenny,Ken, Edward,Michael, Rob, Pablo, if you look at my chart from day one the nurses and CNAs we exceptional, I will refer this place, the entire staff even housekeeping are so nice and polite, I recommend this place for your rehab care..I would return here. If I had to, rather than another rehab acute. Kevin Dias… I almost forgot, Christian..the nurses and CNAs always made me feel welcome, from the beginning, the entire staff. Social workers and beyond were very accommodating to me and im grateful 🙏 I definitely thank everyone, I highly recommend this facility. Terrific staff, extremely caring. Staffing little shy. And their kitchen needs help. Oh, I would still give it a four.

The people here at South Creek like Casie Dodd are so kind and friendly. They have an amazing therapy team. They have lots of fun activities to do everyday. They have nice rooms.

This is a nice place with nice people. Casie is great at helping me get to and from therapy and with my meals. The nursing staff is great. Lots of activities to do.

My experience with these instructors have been incredible. Especially Edward. I would recommend this place to anyone who needs it.

The care has been great. Alec Edward Kaitlyn have been great. They are great helping with the exercises and helping me get stronger and walk.

PT & OT therapists are always available to me.
